- Makes the initial nursing evaluation visit.
 - Continually evaluates the patient's nursing needs.
 - Initiates and coordinates the plan of treatment and necessary revisions.
 - Renders nursing care and performs treatments as ordered by the physician.
 - Reports to the patient's physician and other appropriate personnel pertinent changes in the patient's condition.
 - Supervises and teaches family and personnel.
 - Participates in nursing report.
 - Responsible for patient recertification for home care services.
 - Participates in QA/QI process.
 - Prepares reports and documentation.
 - Is responsible for adhering to all practice standards as they apply to patient care.
 
Job Requirements
- Current RN licensure in state of employment.
 - At a minimum, one (1) year clinical experience. This requirement may be waived for an RN with recent clinical experience as an LPN.
 - Current driver's license and vehicle insurance in state of employment, and the ability to travel within a 50 mile radius on a daily basis.
 - Is responsible for adhering to all practice standards as they apply to patient care.
