Responsible for the coordination and management of patient caseload of Home Care patients. Responsible for the coordination and management of patient caseload of Home Care patients. Performs the primary function of the nursing process assessing planning, implementing, and evaluating care of all assigned patients. Collaborates with Care Coordinator and other members of the patient care team.
- Provides physical and psychological support to patients, friends, and families.
- Provides home care nursing visits, as assigned, according to the policies and procedures and protocols of the agency.
- Makes nursing assessments and reports significant changes in patient condition to the Care Coordinator and/or patient care team members.
- As necessary, completes in-home admission and assessment visits within established parameters and develops/implements individualized patient plans of care.
- Utilizes the appropriate clinical pathways.

- Current Pennsylvania State Registered Nursing license from an accredited school of nursing
- 2 years of Home Health experience
- Case Management experience
- Electronic Medical Charting experience
- Valid Pennsylvania driver license
- Computer literacy is essential
