Job Description

Telecare ACT-Oregon (Assertive Community Treatment) is a comprehensive, community-based program for individuals with serious mental illness (SMI). Services are provided in Clients homes, hospitals or transitional housing.

As a Personal Service Coordinator/QMHA, you will:

• Provides safe, effective, efficient, culturally competent and consumer centered implementation of direct care in accordance with established policies, procedures and standards of care
• Establishes and maintains and co-facilitates a therapeutic relationship with members
• Performs psychosocial interviews with members of program
• Performs service coordination functions of assessing, planning, linking, monitoring and advocating on assigned members
• Participates as active member of Team multidisciplinary staff
• Performs extensive in vivo services as requested/required to members' homes, hospital emergency room(s) and other settings
• Participates in planning and implementation of consumer treatment plans
• Ensures documentation of all services in accordance with standards
• Performs liaison duties with assigned agency
• Collaborates in coordination of consumer services
• Transports persons being served, as needed
• Completes other duties as assigned
• Makes every effort to empower members to improve quality of everyday lives
• Participates in the development of the Individual Service Plan
• Coordinates with member for all needs to live successfully in the community
• Assumes professional responsibility for consumer’s well-being
• Responds to crisis situations

QUALIFICATIONS

• Must qualify as a QMHA in the state of Oregon and have one year of experience in a psychiatric setting is required
• B.S./B.A. or M.S./M.A. in Social Work, Nursing or experience with Substance Abuse or Vocational Counseling or related fields is preferred
• Must have valid Oregon driver's license and access to a car and insurability; must have knowledge and ability to use public transportation
• Knowledge of process of mental illness required
• Applicant must be fingerprinted, other than minor traffic offenses and receive clearance from the Department of Justice
• Specialized knowledge/experience in vocational, housing, or dual diagnosis services preferred
• Bilingual abilities a plus
• Must have completed first aid and CPR training or obtain certification within four weeks of hire
• Must be willing to transport clients in your car

Mental Health Counselor

GENERAL SUMMARY

Under supervision of the Charge Nurse, facilitates and provides psychiatric care of adult, adolescent, and/or child patients by planning and implementing milieu treatment, considering patient needs and therapeutic goals. Ensures quality and safe delivery of care to patients of all ages.

PRINCIPAL DUTIES AND RESPONSIBILITIES

• Functions as a milieu therapist to assigned patients, including collaborating with the nurse therapist to determine a milieu treatment approach, functioning as an active member of the multidisciplinary team, administering an initial psychological assessment, and providing consistent individual counseling focused on treatment goals during hospitalization. Participates in milieu therapy for patients on an individual or group basis. Leads or assists in milieu groups. Incorporates knowledge of each patient's condition and application of various therapeutic techniques; ensures appropriate participation by patients.
• Collaborates with other unit staff in assessing and evaluating patient's mental status. Initiates and maintains a therapeutic relationship with patients. Collaborates in the diagnosis and treatment of patients; shares information and participates in the formulation of treatment plans. Coordinates and implements various aspects of milieu environment to provide a therapeutic experience for patients. Collaborates with other staff members to determine whether a patient represents a risk to himself/herself and/or others. Works with other unit personnel with crisis intervention techniques.
• Provides supportive care to patients, such as participating in conversation, recreational activities, and basic living activities. This may include assisting patients with bathing, eating, and other daily living activities. Sets limits, confronts counterproductive activities and behavior, and reassures and orients patients. Encourages and facilitates appropriate interactions between patients.
• Records all appropriate information on patient charts; prepares regular reports describing each patient's mental status and functioning, as well as significant interactions. Reports and documents overt, covert, verbal, and non-verbal communications by patients.
• Provides support and education to family members and significant others regarding the patient's status. Works with representatives of outside agencies in obtaining or disseminating information on individual patients. Participates in discharge planning and referral, if necessary.

KNOWLEDGE, EXPERIENCE AND OTHER JOB REQUIREMENTS

• The level of knowledge equivalent to that ordinarily acquired through completion of a Bachelor's Degree in psychology or a related field.

WORKING CONDITIONS

• Patient care environment. Periodic risk from combative or abusive patients; occasionally may assist with moving or lifting elderly or handicapped patients or with applying restraints.
• Regular walking and standing during normal course of duties.

Mental Health Technician

Mental Health Technician Position Purpose

Under the immediate supervision of the Registered Nurse and/or Nursing Manager and/or Nursing Supervisor, this position functions as a member of the interdisciplinary treatment team providing patient care. Technicians participate in both the planning and implementing of individual patient treatment plans. They may be responsible for admitting and interviewing patients, record keeping, and conducting appropriate treatment groups.

WORKING RELATIONSHIPS

• Position Reports To: Registered Nurse and/or Nurse Manager or Nursing Supervisor
• Position Supervises: None.

Interpersonal Relationships: As a representative of Springwoods Behavioral Health, all comments, attitudes, actions, and behaviors have a direct affect on the Facility’s image and perceptions of quality service. Interaction with patients, family members, physicians, referral sources, affiliating schools, visitors, etc. must be in a manner that is friendly, supportive, courteous, respectful, cooperative, and professional. This behavior should promote an atmosphere of teamwork, which is congruent with facility standards and guidelines to promote positive relations.

SKILLS, ABILITIES AND COMPETENCIES

• Must have good observation skills.
• Ability to recognize and report changes in behavior of patients to other team members.
• Skill set to work in a fast-paced behavioral health hospital with frequent interruptions.
• Effective oral and written communication skills and knowledge of telephone etiquette is required.
• Ability to interact in a positive manner and to communicate well with patients, families, co-workers, supervisor and other hospital employees.
• Ability to adapt to changes in how information is processed.
• Must be able to exercise good judgment and make recommendations in the best interest of the hospital
• Required visual acuity to provide observation of patients and physical agility to manage patients.
• Proficiency in reading and writing.
• Good organization and time management skills. Strictly adhere to facility time and leave policy.
• Hours may vary based on staffing needs and include day, evening and weekend shifts or combination of shifts.

Mental Health Technician- You must meet these requirements in order to be considered for this position.

POSITION REQUIREMENTS

Licenses or Certifications: Current Arkansas Driver’s License required if approved and designated to transport patients. Current CPR and AED certifications, or must obtain within 90 days of date of hire.

Total education, vocational training and experience: High school diploma or GED with one year nursing experience within a behavioral health or hospital setting or any combination of education and related experience. No substitution will be allowed for the High school diploma or GED. Bachelors Degree in Psychology or related field preferred. Experience working with children (ages 5-12), adolescents (ages 13-18), and/or adults (ages 18 plus) preferred.
